Application Instructions
Faculty Development Workshop 2025
1. Create an account through CFHI
a) Go to www.cfhi.org and on the home page, click on “Apply Now”
b) Create a username and fill out the requested basic personal information.
c) From the drop-down menus choose your program (see below for reference)

2. Fill out the security CAPTCHA code and click on “Create Program”
3. An automatic reply will generate that prompts you to check our email. After following the guidelines outlined in the email you will be able to return to your account to complete the remainder of the program registration
4. Initial Application Forms
a) Complete the “CFHI Application – Faculty Development” (this includes general personal information, such as DOB, address, school)
b) Once the form has been completed and you submit your application, the application will be reviewed. If you have been selected as a participant for the Faculty Development Workshop, more information will be required and further instructions will be provided.
